Optic Disc Pit Surgery

Overview

Optic disc pit maculopathy (OPM) surgery aims to resolve the complications from congenital optic disc pit (ODP), such as fluid accumulation and possible macular detachment. Pars plana vitrectomy (PPV) is a common surgical approach, often performed in combination with internal limiting membrane peeling or gas tamponade.

Widely-Executed Surgical Techniques

Pars Plana Vitrectomy (PPV):

The surgical removal of the vitreous gel (the clear fluid inside the eye). Posterior vitreous detachment is induced to relieve traction on the retina.

Internal Limiting Membrane Removal:

A procedure involving the peeling of the internal limiting membrane on the retina to reduce the chance of traction recurrence and fluid accumulation.

Gas Tamponade:

A technique where a gas bubble is injected into the eye to press the retina back into place and assist with fluid absorption.

Scleral Plug or Flap:

This specialised technique uses tissue or a scleral flap to cover the optic disc pit and reduce the risk of fluid seepage.

Inner Retinal Fenestrations:

This highly sophisticated technique involves the creation of tiny openings in the inner retina. These fenestrations allow fluid to drain into the vitreous cavity, helping to relieve retinal detachment caused by fluid build-up.

Laser Photocoagulation:

An advanced laser treatment used to seal the communication between the optic disc pit and the macula, helping to prevent fluid migration.

Macular Buckle:

This technique involves placing a supportive ring externally over the macula to reinforce it. It is commonly used in cases involving macular detachment.

When Do You Consider Surgery?

Surgery may be considered if you experience significant vision loss due to fluid accumulation or macular detachment, especially when conservative treatments like laser therapy or observation prove ineffective. In such cases, surgical intervention becomes a highly effective option to restore vision and prevent further progression of the condition.
